This informal assessment was found via the Scholastic webpage (see reference). I chose this survey as one that I will someday utilize in my classroom. It provides in depth questions that will really help you gain insight to how each student feels about reading. It also gives variety in the types of questions asked. Some questions are accompanied with faces of different emotion to help the student visual understand what the question is asking and perhaps associate the facial expressions with their own attitude. It also addresses if a student is comfortable with reading from an electronic device, which I find extremely important as technology integration in the classroom is essential. I would consider this survey to be formative in that you could administer this assessment at the beginning of the year, in the middle of the year, and at the end of the year to have a running record of progress or lack thereof, or if the attitude towards reading has changed. I would prefer the student to answer these questions individually, but some questions could be done as a whole class to open dialogue. Students in grades 2nd through 4th should be able to complete this assessment in approximately 20 minutes.
